Other Forms
Applications for a postal proxy vote must be received NO LATER than 5.00pm on the ELEVENTH working day before an election and you must have been appointed as a proxy by that date.
To register using the above form, you need to give an address where you would be living if it were not for your current situation, or an address where you have lived in the past.
